1. Iron man would’ve seen thanos running up to him
2. Iron man wouldn’t slowly get up and raise his fist to snap in thanos’s face. He could’ve easily snapped on the floor or away from thanos
3. Thanos’s sword was destroyed so he couldn’t have used it to kill iron man
4. Thanos doesn’t have the force or a magic sword. He can’t just open his hand and have his sword fly to him

Perhaps many of those people would need the help of a qualified psychologist and time to recover. It would be so unexpected, emotional and scary as if Thanos snapped his fingers in real life right behind you.
